Dancing Daisies Liability Release

In order for your child to participate in a dance class you must print, read and understand, sign, and turn in the paperwork to AriAnna Schaefer. Please understand if the signed waiver is not presented at your child's first class your child is unable to participate in any and all activities. 

Please consider signing the Photo Release for your childs picture to be posted on our blog! This is not required, but greatly appreciated for growth of our business.
